As an employee/worker at company, you agree to take part in the following U- Grow employee wellness program (hereinafter referred to as the "contractor"):
Life Balance Program
The contractor assumes no liability or responsibility for injuries, accidents, or illnesses that occur during the program.
By signing this document, you confirm that you/your employer have the necessary insurance and that you are emotionally and physically able to participate in the employee program.
You are not permitted to forward, reproduce, store, process, duplicate or disseminate any details to any third party. Any images used are the property of the contractor or contract partners and may only be used for these purposes. Use by third parties is not permitted.
Further rights are reserved.
The contents of this program have been researched and formulated to the best of our knowledge and belief. However, no guarantee can be given relating to the accuracy of the information. The publisher/ contractor is not liable for any lasting effects that are directly or indirectly related to the information contained in this program.
Consent and data protection declaration
for the employee wellness program

1.1 These data protection provisions supplement the general terms and conditions and the general data protection declaration of U-Grow GmbH, Kolpingstrasse 8, 4600 Wels (hereinafter: U- Grow) and apply to the employee wellness programs and the associated services.
1.2U- Grow uses personal data, which is necessary for the fulfilment of the contract, only with the voluntary consent of the registered employee of its client (hereinafter referred to as: employee).
2. User account management
To use the employee wellness program, registration of employees is required. If the employee takes part in the programs by U-Grow, U-Grow will create a user account with the employee’s consent. The data will be stored by U- Grow until the booked programs have been processed. If a user account is inactive for more than 12 months, U- Grow treats the account as expired.
3.Data protection declaration of consent
The employee agrees that personal data, including the following,
·Master data (salutation, title, first and last name, email address, telephone number, organization/company, full address, username/display name)·Photo·Password·Server connection of the device·Assigned or last trainer/program·Results and evaluation of the program·Storage of evaluations·Anonymization of the evaluations·Log files with anonymized IP·Response times·Special Needs Data
may be processed by U- Grow for the purpose of contract processing and administration of the user account created on the U-Livewell website.
If the employee has given his consent to U- Grow, s/he can revoke this at any time by sending a letter to U- Grow, Kolpingstrasse 8, 4600 Wels, or by e-mail to email@example.com. The legality of the processing carried out based on the consent up to the point in time of the revocation is not affected.
Legal basis of Data Protection: Consent (Art 6 Paragraph 1 lit a GDPR), fulfilment of a contract, necessary to carry out pre-contractual measures (Art 6 Paragraph 1 lit b GDPR), legitimate interest, in particular defence, exercise, and assertion of legal claims (Art 6 Paragraph 1 lit f GDPR), express consent (Art 9 Paragraph 2 lit a GDPR).
Storage period: Until the end of the business relationship or until the expiry of the guarantee, warranty, statute of limitations, and statutory retention periods (in particular BAO); in addition, until the end of any legal disputes in which the data is required as evidence.
4.Rights of data subjects/right of objection and withdrawal/right of appeal4.1. You have the following rights towards U- Grow with regard to your personal data:· Right to information (Art 15 GDPR),· Right to correction (Art 16 GDPR) or deletion (Art 17 GDPR),· Restriction of processing (Art 18 GDPR),· Right to data portability (Art 20 GDPR),· Right to object to processing (Art 21 GDPR).4.2.Right to object: If the processing of your personal data is based on a balancing of interests (Art 6 Paragraph 1 lit f GDPR: legitimate interests), you have the right to object to the processing at any time for reasons that arise from your particular situation. When exercising your right of objection, we ask you to explain to us your reasons why we should not process your personal data. We will examine your submission and either discontinue or adjust the data processing or advise why we should continue to process your data, we will continue data processing even if it serves to assert, exercise, or defend legal claims.You can object to data processing for the purposes of direct advertising and data analysis at any time. In this case, we will stop processing the data.4.3.Withdrawal: If we have received your consent for the processing of your personal data, you can revoke this consent anytime.
