As an employee/worker at company, you agree to take part in the following U- Grow employee wellness program (hereinafter referred to as the "contractor"):
Life Balance Program
The contractor assumes no liability or responsibility for injuries, accidents, or illnesses that occur during the program.
By signing this document, you confirm that you/your employer have the necessary insurance and that you are emotionally and physically able to participate in the employee program.
You are not permitted to forward, reproduce, store, process, duplicate or disseminate any details to any third party.
Any images used are the property of the contractor or contract partners and may only be used for these purposes. Use by third parties is not permitted.

The contents of this program have been researched and formulated to the best of our knowledge and belief. However, no guarantee can be given relating to the accuracy of the information. 1. General
1.1 These data protection provisions supplement the general terms and conditions and the general data protection declaration of U-Grow GmbH, Kolpingstrasse 8, 4600 Wels (hereinafter: U- Grow) and apply to the employee wellness programs and the associated services.
1.2 U- Grow uses personal data, which is necessary for the fulfilment of the contract, only with the voluntary consent of the registered employee of its client (hereinafter referred to as: employee).
2. User account management
To use the employee wellness program, registration of employees is required. If the employee takes part in the programs by U-Grow, U-Grow will create a user account with the employee’s consent. The data will be stored by U- Grow until the booked programs have been processed. If a user account is inactive for more than 12 months, U- Grow treats the account as expired.
3. Data protection declaration of consent
The employee agrees that personal data, including the following,
may be processed by U- Grow for the purpose of contract processing and administration of the user account created on the U-Livewell website.
If the employee has given his consent to U- Grow, s/he can revoke this at any time by sending a letter to U- Grow, Kolpingstrasse 8, 4600 Wels, or by e-mail to info@u-grow.at. The legality of the processing carried out based on the consent up to the point in time of the revocation is not affected.
Legal basis of Data Protection: Consent (Art 6 Paragraph 1 lit a GDPR), fulfilment of a contract, necessary to carry out pre-contractual measures (Art 6 Paragraph 1 lit b GDPR), legitimate interest, in particular defence, exercise, and assertion of legal claims (Art 6 Paragraph 1 lit f GDPR), express consent (Art 9 Paragraph 2 lit a GDPR).
Storage period: Until the end of the business relationship or until the expiry of the guarantee, warranty, statute of limitations, and statutory retention periods (in particular BAO); in addition, until the end of any legal disputes in which the data is required as evidence.
